Read First – Important Instructions

This form must be completed by a parent or legal guardian.

To ensure we have the correct emergency contact information, please follow these steps carefully:

  1. Begin by entering the parent or legal guardian’s full name and phone number. This contact will be used in case of emergencies during Children's Church.

  2. Next, add your child(ren) and any additional parent/guardian under the "Household Members" section.

  3. Be sure to complete the allergy section for each child. If your child does not have any allergies, please write “N/A.” The form will not submit unless this field is completed.

  4. During church services, please keep your cell phone on vibrate so a Children's Church worker can reach you if needed.

"Add Adult" Add an additional parent/guardian in your household that has permission to check in your child.
"Add Child" Add your additional child or your additional children.
